Immunization Records & Requirements

It is advisable that you print out these instructions and bring them with you to your medical clearance appointments.

Sources for obtaining the immunization history information for the Confidential Health History Form are: parents, family doctor, high school or military. Please bring your documented immunizations record to your medical clearance appointments and use it to complete the Confidential Health History Form prior to your medical appointment. If you are unable to get a hold of these records, then you have the option of getting all the immunization boosters.

Documented proof of immunizations listed below are highly recommended of any person participating in the Education Abroad Program through UC Davis. Documented proof would include: official medical records, signed immunization records from high school, official military records, etc.

These "highly recommended" immunizations are in addition to any EAP or country requirements:

  1. Polio Series
  2. Diphtheria/Pertussis/Tetanus Series: Plus 1 tetanus in the last 10 years.
  3. Measles (Rubeola): 2 immunizations, in lifetime.
  4. Mumps: 1 immunization
  5. German Measles (Rubella): 1 immunization
  6. Tuberculin Skin Test (PPD), done in the USA: Negative result showing testing and reading dates within 6 months of scheduled departure date. As required for selected countries.
    If positive PPD history:
    1. You must have documented proof of the past positive PPD test showing amount of induration.
    2. Documented proof of a normal chest x-ray dated after the positive PPD test.
    3. INH therapy completion certificate (if applicable)
    4. If deemed necessary by physician, you may be required to have another chest x-ray.

Some immunizations are a series of vaccinations that can take up to 2 months to complete. Don't delay starting your health clearance process!
